“Lowest Inpatient Admissions per 1,000 Members” award!

Stones River IPA’s PCMH program was recently recognized for our superior quality of care by receiving the “Lowest Inpatient Admissions per 1,000 Members” award at the annual PCMH Best Practice Forum held this May in Nashville. The IPA’s leadership team would like to take this opportunity to thank all of our PCMH practices for remaining diligent in making improvements to not just the quality of care for our patients, but also for their efficiency of care. Keeping our patients healthier and out of the hospital is the primary goal for our Primary Care Medical Home efforts in Rutherford County. Congratulations to our PCMH practices!
